Out-of-Pocket Payments
Plantation Dental SSI accepts out-of-pocket payments including cash and check, as well as credit cards such as Mastercard, Visa, Discover, and American Express. We also accept CareCredit, a health-care credit card that requires separate approval.
Dental Insurance
For your convenience, Plantation Dental SSI will file your dental insurance if you are able to provide all necessary information to our office. However, our office is not contracted with any dental insurance plans. In other words, your insurance coverage is a contract between you and your insurance company. Additionally, you are responsible for any non-covered or unpaid balances not covered by your dental plan. All copays and deductibles are expected to be paid the day services are rendered.
*An interest rate of 1.5% will be applied to any unpaid balance 90 days old and older.
Plantation Denta-Care Plan
- Plantation Dental SSI proudly offers an in-house plan for patients who do not have dental insurance.Â
- Features of our plan include:
- 1. No preauthorization is required
- 2. No annual maximum
- 3. No deductible
- 4. No waiting period for treatment
- 5. Cosmetic procedures included
- Additional benefits of our plan include:
- Initial comprehensive exam, necessary x-rays, and cleaning (prophylaxis) (D1110) with fluoride
- Six months check-up exam and cleaning (prophylaxis)
- One urgent care exam and x-ray
- Necessary dental procedures completed within 12 months of enrollment will be discounted 10% if paid in full at the time of service by check or cash. If paid in full by credit card, discounted 5% at the time of service. Includes cosmetic procedures such as teeth whitening, veneers, and crowns.
- Bonus: There’s no ID card or member number to bring. All of your membership information will be kept in your personal electronic record. Your effective date is the day you sign up, and your renewal date is the same date every year.
